Why do I need to complete a Course Completion Affidavit ?

Without the Course Completion Affidavit we don't know that you are ready for your certificate of completion - this form also allows you to confirm your personal information and update anything that may not be correct.

Steps 1 :

Log in to your course and select your course from the course overview menu.

Step 2:

To reach the form click on the Course Completion Affidavit and Grade Submittal Form link in the Navigation Menu on the left side of your screen or scroll to the bottom of the course's Welcome page and select it from within the course outline. This will take you directly to the form.

Step 3:

You must check any boxes at the top of the form. Read the statements and check the boxes to confirm your understanding.

Step 4:

Fill out the form with your personal information. (much of it will be pre-filled for you. Please verify your complete address and phone number etc. are correct.)

We require this information to report your completion to the state and email the certificate of completion to you. Some colleges will email the certificate instead - all of this will be automatically processed once you submit your Course Completion Affidavit.

Step 5:

Click the "Submit My Grades" button at the bottom of the form.
After doing that you will be taken to a thank you screen that shows the information you submitted to us.

Follow up:

Our compliance department processes Grade Submittals as soon as they receive them, in the order that they come in. Your Course Completion Affidavit triggers us to confirm your records and submit your course completion to the appropriate office at the state level.

If you took a Designation course that waives the state exam you will be sent the certificate of completion directly and are required by state law to submit it to the state yourself.
